ABSTRACT

The ancient and modern discussions on the treatment of acupoints of the pericardium meridian of hand jueyin and non-acupoints along the meridian for stomach diseases were summarized, and the treatment principle of pericardium meridian for stomach diseases was explored. The relationship between pericardium meridian and stomach was discussed from the three perspectives of heart-stomach correlation, organ-meridian correlation and qi-position correlation. Based on these discussions, the guiding significance of the thoughts of selection effective treatment parts and the theoretical construction for clinical practice was considered and examined.

ABSTRACT

Taking the clinical trial of acupuncture in treatment of postprandial distress syndrome as an example, this paper proposes that the acupuncture clinical trial protocol should be optimized in view of acupuncture prescription, acupuncture frequency and outcomes. Besides, the data quality of acupuncture clinical trial should be improved in consideration of data sharing and electronic data capture so as to provide a reference for the majority of researchers to optimize and implement acupuncture clinical trial.

ABSTRACT

Se presenta el caso de una paciente de 40 años con xantelasma gástrico asociado a Helicobacter pylori, asociación infrecuente en la literatura médica. Se resalta la rareza de esta entidad, las manifestaciones clínicas, el diagnóstico, la terapéutica y la importancia de su relación con Helicobacter pylori. Se realizó una revisión de los aspectos más importantes de estos aspectos. Se concluye que el Helicobacter pylori debe tenerse presente por su carácter de agente etiológico en enfermedades malignas y que deben agotarse todos los medios para su oportuna identificación.


The case of a 40 years old patient diagnosed with gastric Xanthelasma related to Helicobacter pylori is presented. This association is rare to find in medical literature. The following aspects are highlighted: entity rareness, clinical manifestations, diagnosis, treatment and importance of the association with Helicobacter pylori. A literature review on the most important features of these processes has been carried out. It has been concluded that Helicobacter pylori should be seriously taken into consideration because of its aetiology in malignant diseases and that all kinds of investigations should be performed in order to diagnose it in due time.

ABSTRACT

Most of gastric polyps are benign and do not need specific treatment. However, some types have significant malignant potential that can lead to gastric cancer if they are not managed appropriately. The malignant potential depends on the histological type of the polyp, therefore it is important to sample and make biopsies.


La mayoría de los pólipos gástricos son benignos y no requieren tratamiento específico; no obstante, algunos de ellos pueden malignizarse. Si estos pólipos no son tratados pueden ser causa de cáncer gástrico. El potencial maligno depende del tipo histológico del pólipo, por lo que estas lesiones debieran ser siempre biopsiadas.

ABSTRACT

La gastroenteropatía eosinofílica es una entidad inusual caracterizada por infiltración eosinofílica de la mucosa gastrointestinal.Efectuamos un análisis retrospectivo de 10 pacientes diagnosticados como gastroenteropatía eosinofílica entre 2000 y 2010. El diagnóstico histológico se confirmó por infiltración de 20 o máseosinófilos/campo de gran aumento en tracto digestivo superior y/o más de 60 en tracto digestivo inferior. Diez pacientes [edad mediana 10 meses (r 2 meses -10 años)] con predominio masculino (9:1), fueron diagnosticados como gastroenteropatía eosinofílica.La presentación clínica fue: diarrea y deterioro ponderal (4/10); distensión abdominal y deterioro ponderal (2/10) y edema e hipoalbuminemia (4/10). Se administró dieta elemental/hipoalergénicasegún edad de presentación clínica, con remisión sintomática en 4/10. Seis requirieron inducción con corticoides, 5 realizan mantenimiento con budesonide. Dado el incremento de casos de gastroenteropatía eosinofílica en laúltima década, debemos agudizar la sospecha diagnóstica. Un estudio multicéntrico podría colaborar en la definición del mejor enfoque terapéuticoen estos pacientes.

ABSTRACT

Context: Trichobezoars (TB) are bezoars formed by ingested hair. TB are more common among girls and young women. The findings include a hard, mobile and painless abdominal mass in the epigastrium, anorexia, nausea and vomiting, weight loss and reports of thichophagia. Image abdominal studies can confirm the diagnosis. The treatment is based on the complete TB remotion.Case report: M.O.M., 16 years old, female, single, student, born and living in São Paulo. She reported anorexia, abdominal discomfort, halitosis and mild colicky abdominal pain associated with a lump three years ago. On examination: huge, non-tender, mobile, painful, palpable mass in the upper abdomen (13cm) without inflammatory sign. Plain films of abdomen: diffuse radiopaque mass in upper abdomen without gastric bubble. Abdominal ultrasound and computadorized tomography show enlarged stomach filled by heterogeneous material. Endoscopy demonstrates the stomach completely filled with black hair. Once the diagnosis was made, surgical treatment was indicated.

ABSTRACT

Dieulafoy lesion is an abnormally large calibered submucosal artery associated with a minute mucosal defect in the gastrointestinal mucosa. It is a rare cause of profuse, but intermittent gastrointestinal bleeding. The lesion is usually located in the stomach, although it may occur anywhere in the gastrointestinal tract. Dieulafoy lesion is extremely rare in the neonates. We report two newborn infants with a gastric Dieulafoy lesion which was treated by endoscopic epinephrine injection therapy without complication and recurrence.

ABSTRACT

O tratamento da ingestão de agentes químicos corrosivos continua controverso. A incidência desses episódios tem aumentado nas últimas décadas por várias razões. OBJETIVO: Analisar a ocorrência, as complicações e os resultados do tratamento da lesão esôfago - gástrica causada por agentes químicos. MÉTODOS: Foram estudados retrospectivamente 21 pacientes adultos com lesão esôfago-gástrica, causada por ingestão de substância química, admitidos até 23 dias após o episódio, no Serviço de Emergência da Santa Casa de Misericórdia de São Paulo num período de 12 anos. A média de idade foi 32,1 anos e 11 doentes pertenciam ao sexo feminino, as quais mais freqüentemente tinham intenções suicidas. A soda cáustica foi o produto mais ingerido (76,2 por cento), ingestão de ácido muriático ocorreu em três casos (14,3 por cento), amoníaco e ácido sulfúrico em um caso (4,8 por cento) cada. RESULTADOS: As lesões faríngeas e laríngeas estiveram freqüentemente associadas às lesões de esôfago, presentes em 18 casos (85,7 por cento). As lesões esofágicas, gástricas e duodenais foram avaliadas e classificadas por endoscopia. Lesões graves esofágicas ou gástricas estiveram presentes em cinco casos cada. CONCLUSÃO: O tratamento e os resultados foram variados, mas sugeriram que a sondagem esofágica foi prejudicial. A mortalidade global foi 28,6 por cento, mais elevada na lesão esofágica grau 3.

ABSTRACT

A gastropatia hipertensiva portal (GHP) é uma patologia descrita recentemente sendo uma importante causa de hemorragia digestiva alta. Sua principal forma de apresentaçäo é a perda crônica de sangue pelo trato gastrointestinal, algumas vezes com intensa anemia. Todavia, näo säo raros os casos de hematêmese e melena com instabilidade hemodinâmica. O tratamento clínico adequado e preventivo pode evitar deterioraçäo da funçäo hepática. Através deste manuscrito o autor define GHP, descreve as classificaçöes propostas e reve os aspectos fisiopatológicos, histológicos e endoscópicos da GHP. A sua correlaçäo com os níveis de pressäo portal e a funçäo hepática também säo abordados bem como suas manifestaçöes clínicas e tratamento.

ABSTRACT

Se estudiaron veintinueve (29) casos de pacientes pediátricos, que fueron llevados al Servicio de Gastroenterología del Hospital General del Oeste por presentar diversos signos y síntomas del aparato digestivo, siendo el predominante dolor abdominal recurrente. A todos se les realizó estudio clínico, endoscopia del tracto digestivo superior, prueba de la ureasa y biopsia gástrica. Sus edades estaban comprendidas entre los cinco meses y los catorce años. Dieciséis pacientes fueron del sexo femenino y trece del masculino. En dieciséis se hizo el diagnóstico endoscópico de gastritis y la prueba de la ureasa resultó positiva en doce. Desde el punto de vista histopatológico en doce (12) casos se encontró evidencia inequívoca de helicobacter pylori, observándose en la mayoría de ellos una tríada histopatológica constituida por: 1) gastritis crónica superficial activa con plasmocitos e hiperplasia regenerativa típica; 2) hiperplasia vellosa, y 3) dilatación de criptas y/o glándulas. De los doce casos con presencia inequívoca de Helicobacter pylori, nueve presentaron la prueba de la ureasa positiva
